Scheduling a Session
In order to best make use of your time, we ask that all clients print and complete their intake paperwork from this website prior to their first appointment. If this is not possible, we would ask that all new clients come 15-20 minutes prior to their initial appointment time in order to complete this important information. Complimentary coffee/water and a healthy snack option is available to you while you wait. We would also ask for those with children coming as identified clients, that an additional trusted caregiver attend the initial session in order to remain with the child during the parental interview portion of the session. This first session is the longest at an estimated 90 minutes and can prove to be a difficult wait for even the most patient child.
Payment for co-pays, deductibles, and other out-of-pocket fees will be due at time of service delivery. Any payment refused to be paid by insurance providers are ultimately the out-of-pocket responsibility of the client.
Clients may pay out-of-pocket if they so choose.
It is also suggested that new clients bring a comprehensive listing of all current medications and medical diagnoses to this initial session. This is imperative as the division between one’s physical and psychological well-being are intricately connected with one consistently impacting the other.
Following the completion of this interview, the therapist will engage in the completion of a collaborative treatment plan with the client. The client is actively engaged in this process so that they will understand each person’s role and expectations in their roadmap for healing.
Our office also allows for the provision of short-term in-home visits as well as phone or other out-of-office consults to allow for patient flexibility and convenience per an out-of-pocket fee.
We also serve clients from a satellite location in Sioux Center.
